Entry and doorway

Door swings clear; threshold is visible; no cords across the path.

Night route and lighting

Path to the toilet is lit enough to walk without rushing.

Dark spots, glare, or risky leaning.

Floor traction

Mats are cleaned on a schedule; water does not pool after use.

Sliding mats or slick tile.

Shower or tub entry

Stepping over the tub rim is safe, or a transfer plan is in place.

High tub wall without a bench plan.

Shower seating

Standing through a shower is safe, or a seat fits the stall.

Fatigue or dizziness while standing.

Handholds

Stable handhold locations are planned; suction-only bars are not the body-weight plan.

Wobbly bars or reaching with nothing solid.

Toilet height

Sit-to-stand at the toilet feels manageable, or a riser is installed.

Hovering or struggling to stand.

Toilet-side support
